1.
Thank you for your interest in the MECC Healthy Hero Award! We encourage you to use the fields below to outline your case study.
Please avoid using any resident names, however, you are very welcome to use your organisation's name!
To begin, tell us about your current role: *
3.
Tell us how you use MECC in your role
(are you a MECC Champion, do you and your team talk about MECC conversations, do you usually signpost residents to services?) *
4.
Please provide a specific example when you found yourself in an opportunistic conversation, outside of the usual remit of your role, where you put MECC into practice. Could you please describe the opportunity you spotted to have the MECC conversation, the conversation itself and the engagement from the person you were talking with? This could be signposting a resident to health and wellbeing services, or referring a neighbour into housing and employment services, for example. *
5.
What was the thing that stood out about this conversation to you? What did you learn from the experience? Did you find out what happened after your conversation with the resident?
We understand that sometimes we don't know how far a resident, neighbour, or friend may take the advice we give. It is one of the difficult things to measure with MECC! If that is the case, explain the situation, the possible outcome, or how the person responded to the information! The more detail the better! *
6.
Do you consent to the MECC team using this example on their website, in promotional material, and/or as an example for trainees? *
