1. Create

New survey

To get started with a new survey:

  1. Enter the New Survey page from the main menu.
  2. Choose New to start from scratch, Copy to duplicate an existing survey, or Template to start from a pre-built layout.

Add a question

To add a question to your survey, click the arrow button Arrow button to open the options menu and select Add Question. In the Add Question form, start by choosing the question type. There are several question types to choose from:

  • Multiple Choice, Single answer – respondents select one of the several options
  • Multiple Choice, Multiple answers (Checkboxes) – respondents can select multiple options
  • Multiple Choice, List (Dropdown menu) – respondents select one option from a dropdown menu
  • Open Text, Single Line – respondents provide short text answers
  • Open Text, Paragraph – respondents provide longer text answers
  • Open Text, Multiple Text Boxes – respondents provide multiple short answers
  • Matrix, Single answer per row – respondents select one option per row from a two-dimensional grid
  • Matrix, Multiple answers per row (Checkboxes) – respondents select multiple options per row from a two-dimensional grid
  • Matrix, Rating/Likert – respondents rate multiple options from a two-dimensional grid
  • File Upload – respondents upload one file (only available on Pro and Premium plans)

When you’ve selected the question type, enter the question text and the possible responses to the question. You can add basic HTML tags to create links, bold, italic, and underline text. Check Answer required to ensure respondents can’t skip the question.

Add a logo

To add a logo to your survey:

  1. Click the arrow button Arrow button next to the survey header and select Edit Header.
  2. In the form, click the Add image to upload your logo.

Add a page break

If your survey is long, you can add page breaks to organize it in multiple pages. Click the arrow button Arrow button where you'd like the page break and select Add Page Break.

Add other content

From the arrow menu, select Add Content to insert text, headings, or images.

Edit questions and content

Use Edit, Add, Duplicate, Delete, or Move to manage each item in your survey.

  • Edit – To edit the selected item.
  • Add – To add another question or content below the item.
  • Duplicate – To duplicate the item.
  • Delete – To delete the item.
  • Move – To move the item up or down.

Question options

For each question, you can configure the following options depending on the question type:

  • Answer required – To make sure respondents answer a question.
  • Randomize choices – To randomize the order of the choices for each respondent (only available on Multiple Choice and Matrix questions).
  • Limit the number of choices – To limit the number of choices a respondent can select (only available on Multiple Choice and Matrix questions).
  • Validate answer format – To make sure the respondent enters a number, email address or date (only available on Open Text questions).
  • Each column can only be selected one time – Ensure each column is selected only once. Useful for ranking options in Matrix questions.
